Important advice on using the form
1. Click on the purple part of the screen inside the online entry form.
2. Scroll down to complete the form by clicking the SUBMIT button
3. If you have difficulty finding the submit button, please see this helpful advice
… the online form was cut off at the bottom before the submit button appeared. To see the submit button I changed the magnification of my screen from 125% to 100%.
